stomachache, toothache, headache應否加冠詞’a’？

問：我從文法書上得知, 像stomachache, toothache 這樣一些表疾病的名詞是用作不可數名詞的。但我的學生卻從詞典中查到下面的句子, 我不知如何解釋:
I have a toothache.

答：除headache 外, 其他由-ache 構成的表疾病名稱的名詞一般都用作不可數名詞。但是, 當它們表示疾病的一次發作或疼痛時, 在美式英語中常常加不定冠詞。請比較:

1. Nuts give toothache. 咬堅果會牙疼。
2. On and off she suffered from a stomachache. 她一陣一陣地胃痛。
